Program Scope
Sunshine Pathways Foundation operates a Transitional Housing Program at 1006 10th Street in West Palm Beach, FL — the project's existing site. Today the program serves 16 residents; planned expansion increases that capacity to 31 beds, nearly doubling the program's reach.

The Transitional Housing Program reduces recidivism by combining stable housing with active case management. Residents receive a safe place to live while working with case managers on employment, sobriety, family reintegration, and the practical steps that lead to long-term independence — not a perfect history.

Eligibility
The transitional housing cohort is defined by connection to criminal-legal involvement and a demonstrated willingness to engage with the program. Eligibility is determined at referral — not all applications are accepted, and placement depends on current availability.
Individuals released from county jail, state prison, or federal institution with no safe housing.
Individuals referred by Drug Court, probation or parole officers, or partner reentry programs.
Participants must agree to maintain sobriety for the duration of their stay in the program.
Willingness to comply with house rules, curfew requirements, and active case management participation.
Emergency placements are accepted for reentry — partner agencies with an urgent situation should contact us directly rather than submit through the standard referral form.
Intake Flow
Once a referral is received, the program's intake coordinator moves through a structured flow with target turnaround times at each stage.
Partner agency submits the referral through the partner portal, with urgency, client info, and referrer contact.
Within 24 hours the intake coordinator reviews the referral against availability and population fit, with a confirmation to the referrer.
Phone or in-person assessment: willingness to follow house rules and commitment to the case plan — assessed, not screened out by background.
Placement confirmed within 48–72 hours of referral. Onboarding with case manager begins on arrival.
