Sunshine Pathways Foundation's Reentry Case Management program serves individuals with a justice-system connection across Palm Beach County, FL — coordinating weekly case-manager check-ins, supervision checkpoints, and warm hand-offs between the county jail (PBSO), state prison (FL DOC), the federal BOP, and partner reentry programs.

Each participant meets their assigned case manager for a structured weekly check-in covering housing, employment, sobriety, family, and benefits domains.

Within 24 hours of referral, the intake coordinator confirms receipt, assigns a case manager, and schedules the initial screening.

Program Mission

Reentry Case Management is the connective tissue of SPF's community-based reentry work — coordinating supervision checkpoints, employment and benefits navigation, family reintegration, and warm hand-offs to housing so that no client falls through the seams between county jail release and a stable first 90 days.

Intake is open to anyone with a justice-system connection in Palm Beach County, FL — regardless of whether they enter SPF's housing programs. Referrals come directly from the Palm Beach County Sheriff's Office (PBSO) pretrial/release unit, Florida Department of Corrections (FL DOC) transition coordinators, federal Bureau of Prisons (BOP) community programs, Circuit 15 probation and parole officers, the FL-15 AND-1 drug court, and partner reentry programs; self-referrals are welcomed via /contact.

Who This Program Serves

The reentry case-management cohort is defined by a justice-system connection in Palm Beach County and a demonstrated willingness to engage with structured weekly check-ins. Eligibility is determined at referral — not all applicants are accepted, and case-management openings depend on current case-manager caseload capacity.

Justice-System Connection in Palm Beach County

Active or recent connection to county jail (PBSO), state prison (FL DOC), federal incarceration (BOP), or community supervision (probation, parole, FL-15 AND-1 drug court) within Palm Beach County.

Documented Release / Supervision Status

Release date and supervising officer (probation, parole, pretrial) documented at referral; the assigned case manager coordinates directly with the supervising authority to align case-plan milestones.

Weekly Case-Manager Engagement

Commitment to weekly structured case-manager check-ins covering housing, employment, sobriety, family reintegration, and benefits navigation.

Palm Beach County Residency

Plan to reside, or willingness to re-settle, within Palm Beach County, FL for the duration of case-management engagement. Geographic scope is Palm Beach County only.

Urgent Cases

Agencies with an urgent case-management need — immediate post-release coordination, supervision escalation, or imminent housing risk — should contact us directly rather than submit through the standard referral form.

Intake Flow

What Happens After a Referral

Once a referral is received, the program's intake coordinator moves through a structured flow with target turnaround times at each stage.

Referral Received

Partner agency, supervision officer, or self-referral submitted with urgency, client info, and referrer contact through the partner portal, /contact, or /volunteer.

Acknowledged

Within 24 hours the intake coordinator reviews population fit and case-manager caseload, assigns a case manager, and confirms engagement back to the referrer.

Client Screening

Phone or in-person assessment covering supervision status, housing stability, employment trajectory, sobriety, and case-management goals — assessed against the program's population criteria.

Placement & Onboarding

Individualized case plan finalized within the first week; weekly case-manager check-ins, milestone tracking, and warm hand-offs to housing or partner programs begin on day one.
